Aurora Health Care partnered with 30 local health departments, including the city of Milwaukee's, to survey residents on their health status and habits and identify areas of greatest need.

The report includes data on behavioral and lifestyle habits of the adult population, the prevalence of risk factors and disease conditions. Where appropriate and available, health data of residents is compared to state and national measurements.
